Administrative errors are the most common reason for delays. Before applying, register as a Work Seeker with the Department of Employment and Labour, and prepare the following:
- Original ID: a 13-digit bar-coded ID or valid passport.
- Form UI-19: your employment record, completed and stamped by your employer.
- Form UI-2.8: banking details, stamped by your bank.
- Stamped bank statement: not older than 3 months.
- Form UI-2.11: a key application document.
- Forms UI-2.7 and UI-2.3: primary application forms.
- Form UI-4: a follow-up form for ongoing claims.
- Medical or birth certificates: for illness or maternity claims specifically.
The UI-19 form is your most powerful tool. Your employer is legally obligated to provide it — refusal is an offence that can lead to a fine or imprisonment. If they refuse, remind them of their legal duty or report them to your nearest Labour Centre.
